Joseph Giachinta, 58, was performing maintenance on the forklift at his business, Mid-Hudson Concrete Products, located on Route 9 in Philipstown, when the hoist and jack system holding the forklift up fell, pinning Giachinta under the heavy piece of machinery. A company employee found him late Sunday morning and called 911, according to the sheriff's office.

Giachinta was pronounced dead when emergency responders showed up at the scene, officials say.

The Putnam County Coroner’s Office has ordered an autopsy to determine the exact cause of death, according to the report.
